The Evolution of Palatal Expansion

The first expanders emerged in the 1860s, using a simple screw placed between the upper teeth to separate the midpalatal suture—the line running down the roof of your mouth. This mechanism proved a powerful idea: the upper jaw is not as rigid as once thought.

Decades later, RPEs became routine. While effective for younger patients, they applied excessive force to the teeth, causing significant dental movement (tipping) rather than pure skeletal change, especially in teens and adults whose bones are denser.

Introducing Bone-Anchored Expansion: MSE and MARPE

The true revolution came with the arrival of bone anchors, known as TADs (Temporary Anchorage Devices) or mini-implants. We reimagined expansion by combining these mini-implants with the RPE design to anchor the device directly to the bone. He called this the Maxillary Skeletal Expander (MSE).

This bone anchorage creates a biomechanical shift, allowing us to push directly on the skeletal structure rather than the teeth. By applying the forces on the bones, the teeth are safer, and the procedure is more predictable and effective for older teens and many adults.

The “Opening the Book” Metaphor

Remember the metaphor of your upper jaw as a hardcover book? Let’s call the spine the midpalatal suture. When we activate MARPE or MSE, we gently “open the book,” separating the right and left maxillary bones. As the maxilla widens, related sutures and growth plates respond. We see increased dimensions in the bone structure of your upper jaw, nasal passage, and nasopharynx. After we achieve the desired width, new bone fills the gap, stabilizing your result for the long term.

Who is a Candidate for MARPE/MSE?

If you notice any of the following, maxillary expansion may be a beneficial treatment option:

  • A narrow smile or crowded front teeth.
  • A crossbite involving the upper teeth tucking inside the lower teeth.
  • A jaw that shifts when you close your mouth.
  • A deep groove on the roof of the mouth (high-arched palate).

  • A V-shaped dental arch, as is often seen in long-faced individuals.

While children and young teens with early crowding or posterior crossbites consistently benefit, many older teens and adults whose upper jaw never fully developed can also achieve stable width without surgery.

You may encounter marketing for various devices, such as the Facegenics Midface Expander (FME). While branding can be compelling, serious clinicians prioritize systems backed by robust, peer-reviewed literature. We focus on techniques such as custom 3D-printed MARPEs, which leverage established biomechanics of MSE to provide precise control and strong clinical data for predictable outcomes.
