Stock and Patient Images This website includes stock photography models (Models) who are not actual patients of Phillips Family Orthodontics. Phillips Family Orthodontics has obtained the rights to use the photographs and use of the photographs is in compliance with the terms of those license agreements. Photography of patients is used with the patient’s express consent, and is not to be taken, used, or repurposed for any purpose or in any capacity. Patients have the right to retract their consent at any time, and can do so by notifying us directly via phone at 858.578.1822 or email at hello@phillipsfamilyortho.com.
The models and patients in any image do not personally endorse Phillips Family Orthodontics, or any products, services, causes, or endeavors associated with, or provided by, Phillips Family Orthodontics. The context in which the photographs are used on this website is not intended to reflect personally on any of the Models shown in the photographs. Phillips Family Orthodontics respective officers, directors, employees, agents and/or independent contractors assume no liability for any consequence relating directly or indirectly to the use of the photographs showing the models or patients on this website.

4. Patient Harbor Portal & Online Accounts Phillips Family Orthodontics provides access to an online patient portal (“Patient Harbor”) for account management, payments, and scheduling.
Account Security: You are responsible for maintaining the confidentiality of your account credentials and for all activities that occur under your account. You agree to notify us immediately of any unauthorized use of your account.
Information Accuracy: You agree to provide accurate, current, and complete information when updating health history, addresses, and payment methods within the portal.
5. Online Scheduling & Rescheduling Our website and patient portal offer self-scheduling features.
Rescheduling Limits: To ensure efficient operations for all patients, our portal utilizes a reschedule credit system. You may reschedule appointments freely within your allotted credits. Once those credits are exhausted, you must contact our office directly by phone to make further schedule changes.
Staff Adjustments: We reserve the right to modify, confirm, or cancel any appointments made through the online scheduling widget. Our staff will communicate any necessary adjustments via phone or text.
6. Financial Agreements & E-Signatures The patient portal allows you to view financial agreements, accept treatment plans, e-sign documents, and make payments.
Electronic Signatures: By utilizing the e-sign feature for financial agreements and informed consents, you agree that your electronic signature is the legal equivalent of your manual signature.
Payments: You authorize Phillips Family Orthodontics (and our third-party payment processors) to charge the credit card, debit card, or bank account provided for down payments, remaining balances, or custom amounts as selected by you. You are responsible for ensuring your payment methods remain current to avoid interruptions in treatment.
